The process:

1. Notification of the insured person's death.

How do I report a death?

Contacting State Farm and reporting the death of the insured person starts the Life Insurance Claim process.

Please note that although anyone may report the death, the agent of record will contact the beneficiary designated in the policy.

The following information about the insured person will need to be provided:

• Insured's full name
• Nickname (if any)
• Date of birth
• State of residence
• Policy number(s), if known
• Date and cause of death
• Name of person reporting the claim
• Relationship of the deceased
• Daytime and evening phone numbers
• Additional information/comments

2. Claim processing

After the Life Claim Operation Office receives the notification of death, the agent of record will be notified to contact the beneficiary designated in the policy and to obtain forms necessary to process the claim (for example, certified death certificate, W-9 form).

The agent of record will also be able to provide the status of the claim.

The Life Claim Operation Office may request additional information in order to process the claim.

3. Resolution

A resolution of the claim will be made after a Life Claim Operation Office has received and reviewed all of the necessary forms and documentation.
